Abstract

The Maternal Mortality Rate (MMR) AND the Infant Mortality Rate (IMR) are one of the topics discussed in the 2020-2024 National Medium Term Development Plan (RPJMN). According to the survey results that have been conducted, the high and low maternal mortality rate (MMR) and infant mortality rate (IMR) in a country can be seen from the ability to provide quality and comprehensive obstetric services. One of the government's efforts to reduce MMR and IMR in Indonesia is by providing Continuity Of Care (CoC) care. Continuum of Care is a model of midwifery care in which patients and health workers are involved cooperatively from pregnancy, childbirth, postpartum, newborn services and family planning services. This research uses the case study method (Case study) intensively. Data analysis was used as a technique in making narratives in the results of observations or results of this COC study targeting third trimester pregnant women with physiological pregnancies at 36-40 weeks' gestation. This research provides an opportunity for authors to apply the knowledge gained in practice, add insight, experience and develop themselves in providing Continuum of Care midwifery care to mothers starting from pregnancy, childbirth, postpartum, newborns, and family planning as well as being able to analyze situation and understand the immediate action to be taken.
