Abstract
Uncontrolled hyperglycemia in patients with diabetes mellitus will cause disruption of blood flow to the periphery due to the accumulation of sugar products in the blood and abnormalities of blood vessel endothelial cells, thus disrupting the process of impulse transmission activity by nerves and damage to blood vessel walls. Complications that often arise cause disruption/decrease in peripheral blood vessel flow, which will give rise to nursing problems of ineffective peripheral perfusion. One effort to improve blood circulation in the legs is to perform Buerger Allen Exercise, using the assessment indicator, namely the ankle brachial index.
The purpose of this case study is to determine how the application of Buerger Allen Exercise in increasing ABI values in patients with type 2 diabetes mellitus with ineffective peripheral perfusion nursing problems at RSU Muhammadiyah Ponorogo. This study uses the problem solving method with a nursing care approach. Nursing care was carried out for 5 days, namely on July 12-16, 2024.
Based on the analysis of data obtained from the problem of ineffective peripheral perfusion nursing, after nursing intervention in the form of Buerger Allen Exercise in the problem experienced by Mrs. Y, the ABI value was monitored to increase, which was originally on day 1, namely 0.79 (moderate) and on the 5th day to 1.0 (normal).
After nursing intervention with Buerger Allen Exercise, the ABI value increased. With the increase in the ABI value, peripheral perfusion also increased followed by the achievement of the desired outcome criteria. So the problem of ineffective peripheral perfusion nursing in Mrs. Y was resolved. It can be concluded that Buerger Allen Exercise can increase ABI values in patients with type 2 diabetes mellitus with ineffective peripheral perfusion nursing problems.
