Mental Nursing Care of Schizophrenic Patients with Auditory Hallucinations

Auditory hallucinations are one of the main symptoms experienced by patients with schizophrenia, characterized by the perception of hearing voices that seem to originate from outside the patient's own mind. These voices are typically threatening or provocative in nature and often encourage the patient to engage in harmful behaviors toward themselves or others. The aim of this scientific paper is to provide an overview of mental health nursing care for patients with schizophrenia who experience sensory perception disturbances: auditory hallucinations. This includes the assessment process, data analysis, diagnosis formulation, intervention planning, implementation, and nursing evaluation.
The nursing care was conducted over a period of four days in February 2025 at the Gatotkaca Room of Dr. Arif Zainudin Mental Hospital, Surakarta. The method used was a problem-solving approach through the nursing process in a case study of a schizophrenia patient with auditory hallucinations. The assessment findings indicated that the client experienced auditory hallucinations.
Assessment results indicated that the client frequently heard whispering voices urging them to engage in harmful behaviors, particularly when alone and daydreaming. Based on this condition, the client was identified as experiencing sensory perception disturbance: auditory hallucinations.The client was provided with nursing interventions using Implementation Strategies 1 to 3. In the first implementation, the client was able to control the hallucinations using the rebuking technique. In the second implementation, the client began to redirect attention away from the hallucinations by talking to others and listening to music on the television. In the third implementation, the client applied deep breathing relaxation techniques and participated in scheduled activities as methods to manage hallucinations. All strategies were carried out with guidance and reinforcement from the nurse. As a result, the client was able to independently and effectively manage the hallucinations, and the evaluation showed that the problem was resolved.

This nursing care intervention is expected to enhance the client’s knowledge and ability to manage their hallucinations effectively.
