NURSING CARE FOR ELDERLY PEOPLE WITH PERSONAL HYGIENE DISORDERS WITH NURSING PROBLEMS
OF SELF-CARE DEFICIT (BATHING)
(Case Study at UPT Social Services Tresna Werdha Magetan)

Self-Care Deficit is a disorder of the ability to perform self-care activities such as bathing, grooming, eating, dressing. Self-care deficit is a condition in which a person experiences an abnormality in the ability to perform or complete daily life activities independently. This condition can cause various symptoms such as weakness, musculoskeletal disorders, neuromuscular disorders, psychological or psychotic disorders, decreased motivation or interest. The purpose of writing this scientific paper is to carry out nursing care for the elderly who experience personal hygiene disorders with self-care deficit nursing problems (bathing). The method used is a case study with a nursing process approach including: assessment, nursing diagnosis, planning, implementation, and evaluation.
The results of the assessment showed that the client looked dirty, the client changed clothes every 3 days, the client looked untidy and disheveled, the client looked very smelly, the client looked itchy on his feet. The nursing diagnosis that was established was a self-care deficit related to decreased motivation or interest marked by refusing to do self-care. The implementation of nursing was carried out for 5 days starting from April 16, 2025 to April 20, 2025. Actions included identifying the type of assistance needed, monitoring body hygiene (eg. hair, mouth, skin, nails), monitoring skin integrity, providing toiletries (eg. soap, toothbrush, shampoo), providing a safe and comfortable environment, facilitating brushing teeth, explaining the benefits of bathing and the impact of not bathing on health. The evaluation results showed improvements, namely the client looked clean, the client changed clothes once a day, the client looked neat, the client did not smell. The nursing care provided was proven to be effective in overcoming self-care deficits (bathing).
