CKD is a decline in kidney function that is unable to maintain fluid balance. One of the health problems that many patients experience CKD is thirst. Fluid restrictions are applied to hemodialysis patients to maintain fluid balance. This case study aims to provide nursing care to CKD clients with hypervolemia nursing problems.
Nursing care was provided for 5 days. The method used was a nursing process approach. The assessment revealed the client complaining of shortness of breath and swelling in both lower extremities. Based on this assessment, the nursing diagnosis was hypervolemia.
The client nursing intervention is fluid monitoring. Therefore, the nursing action is therapy ice cube’s. On the last day, before the researcher implemented the therapy, The client said his thirst scale was at level 5. After being given therapy, the client said there was a fresh sensation and the client's thirst intensity was at level 3.
On July 16, 2025, the client complained of shortness of breath and swelling of both lower extremities. The client had edema in both legs, intake > output, and Ht decreased. For 5 days, researchers implemented the therapy ice cube’s, the results of the client's thirst intensity scale were level 3. Furthermore, the evaluation on the last day provided education to the client's family regarding the fluid restriction program.
The conclusion is that CKD patients are expected to control their thirst so that there is no excess fluid which causes the client's body to develop edema by applying therapy ice cube’s independently.
