Hypertension is a highly prevalent non-communicable disease in Indonesia and a major risk factor for cardiovascular disorders. The role of the family, particularly as caregivers, significantly influences the success of hypertension management, yet often leads to caregiver role strain.

This study employed a case study design using the nursing process approach (assessment, diagnosis, planning, implementation, and evaluation). The subject was the family of Mr. Y, who has a family member with hypertension. Data were collected through interviews, observation, and vital sign assessments. The intervention included caregiver education on a low-salt diet, smoking reduction, blood pressure monitoring, and emotional support.

After four visits, the family demonstrated improved knowledge regarding the relationship between lifestyle and hypertension. The patient reduced cigarette consumption and began trying low-salt meals. The caregiver showed better role adaptation, although concerns about patient adherence remained.

Caregiver education effectively enhanced family knowledge and involvement in hypertension care. However, sustainable behavioral change requires continuous support. Emotional encouragement and innovation in dietary management are essential to reduce caregiver role strain.
