ABSTRACT
NURSING CARE FOR PATIENTS WITH PULMONARY TUBERCULOSIS
WITH NURSING PROBLEMS OF
INEFFECTIVE BREATHING PATTERNS
(Case Study in the Asoka Ward of Harjono Ponorogo Regional Hospital)

Ineffective breathing pattern is a condition in which an individual experiences
actual or potential loss of ventilation associated with changes in breathing patterns. This
case study aims to provide an overview of nursing care for a patient with pulmonary
tuberculosis (TB) presenting with an Ineffective Breathing Pattern in the Asoka Ward of
Harjono Ponorogo Regional Hospital.
This nursing care was carried out using the nursing process approach, which
includes assessment, data analysis, nursing diagnosis, intervention, implementation, and
evaluation. The care was provided in the Asoka ward of Harjono Ponorogo Regional
General Hospital for 3 days, from July 18-20 2025.
The assessment revealed that the patient complained of shortness of breath and
coughing up blood-tinged phlegm, consistent with the symptom theory of Pulmonary
Tuberculosis. The established nursing diagnosis was an ineffective breathing pattern
related to respiratory effort obstruction. Nursing interventions provided included
monitoring the breathing pattern (rate, depth, respiratory effort), monitoring for
additional breath sounds (e.g., grunting, wheezing, dry rhonchi), monitoring sputum,
placing the patient in a semi-Fowler's position, providing warm fluids, performing chest
physiotherapy, administering oxygen, encouraging a fluid intake of 2000 ml/day, and
teaching effective coughing techniques. The evaluation revealed that the nursing problem
of the ineffective breathing pattern was resolved after three days of intervention.
Nursing care for pulmonary tuberculosis patients with nursing problems of
ineffective breathing patterns is expected to provide an overview for nurses, especially
in the treatment of pulmonary tuberculosis patients with nursing problems of ineffective
breathing patterns.
