NURSING CARE FOR ELDERLY AGERS WITH ACTIVITY DISORDERS AND NURSING PROBLEMS RISKING FALLS
(Case Study at the Tresna Werdha Social Services Unit, Magetan Regency)

Risk for falls is one of the most common problems among older adults due to the aging process, which leads to decreased physical function, balance, and muscle strength. This condition may increase the likelihood of injury, loss of independence, and a decline in the quality of life of older adults. The purpose of this case study was to implement nursing care for an older adult experiencing activity impairment with the nursing problem of risk for falls at UPT Pelayanan Sosial Tresna Werdha Magetan Regency.
This study used a case study method with a nursing process approach, including assessment, nursing diagnosis, planning, implementation, evaluation, and documentation. The participant was an 83-year-old female older adult (Mrs. K) who experienced activity impairment with a nursing problem of risk for falls. Data were collected through interviews, observation, physical examination, and documentation review.
The assessment results revealed complaints of knee stiffness, aching pain, and muscle cramps, a history of previous falls, impaired balance, and decreased lower extremity muscle strength. The Morse Fall Scale (MFS) assessment score was 60, indicating a high risk for falls. The nursing diagnosis established was Risk for Falls. Nursing interventions focused on fall prevention through risk factor identification, mobility monitoring, environmental safety management, fall prevention education, and assisted ambulation exercises. After seven days of nursing implementation, the participant demonstrated improvement, as evidenced by better balance, more stable walking ability, increased caution during activities, and the absence of fall incidents throughout the care period.
In conclusion, the implementation of comprehensive nursing care with fall prevention interventions was effective in reducing the risk for falls and improving the safety and activity performance of older adults.
