Abstract
Hemorrhagic stroke results in rupture of cerebral blood vessels, causing
decreased saturation values and ineffective brain tissue perfusion. Brain tissue
perfusion can be improved with non-pharmacological therapy, which can
facilitate increased cerebral blood flow, thereby triggering an increase in oxygen
saturation values. One of the interventions in stroke patients in increasing oxygen
saturation values is by providing a semi-Fowler position. The purpose of this
study was to determine the application of the semi-Fowler position to
hemodynamic changes in hemorrhagic stroke patients who experienced nursing
problems of ineffective cerebral perfusion risk.
Nursing care was carried out on Mr. T who experienced a risk of
ineffective cerebral perfusion with a medical diagnosis of CVA ICH on August 19-
23, 2024 in the Ahmad Dahlan Room, Muhammadiyah Ponorogo Hospital. The
method used was the nursing process approach. From the results of the
assessment, it was found that the patient was conscious but tended to sleep and
looked restless, the patient experienced nursing problems of ineffective cerebral
perfusion risk due to the patient's high blood pressure of 203/115 mmHg and
cerebral aneurysm. The implementation carried out to overcome the problem is
the management of increased ICP by implementing the semi-Fowler position.
Management of increased ICP by implementing the semi-Fowler position
effectively improves patient hemodynamics and cerebral perfusion increases after
being implemented for five consecutive days. From the evaluation results, it was
found that the level of consciousness increased, anxiety decreased, consciousness
improved, systolic blood pressure improved, diastolic blood pressure improved,
and nerve reflexes improved, so it can be concluded that the risk of ineffective
cerebral perfusion does not occur.
