APPLICATION OF HEAD UP 300
POSITION WITH MEAN ARTERIAL
PRESSURE (MAP) IN CEREBROVASCULAR ACCIDENT (CVA)
PATIENTS WITH NURSING PROBLEMS RISK
OF INEFFECTIVE CEREBRAL PERFUSION
At the Rumah Sakit Umum Darmayu Ponorogo
Cerebrovascular accident (CVA) can cause the risk of ineffective cerebral
perfusion caused by thrombus which can originate from an atherosclerotic plaque
or blood can clot in areas of stenosis, and where blood flow is slowed or
turbulence occurs. If left untreated, intracranial pressure will increase and
hemodynamic changes occur.
The purpose of this case study was to analyze the application of the head up
position of 300 with mean arterial pressure (MAP) in cerebrovascular accident
(CVA) patients with nursing problems at risk of ineffective cerebral perfusion in
the Flamboyan 2 room, Rumah Sakit Umum Darmayu Ponorogo. Data collection
was carried out by means of participant observation and interviews. The case
study was carried out for 5 days from 16 November to 20 November 2024 in a
cerebrovascular accident (CVA) patient with nursing problems with the risk of
ineffective cerebral perfusion with the implementation of the head up 30° position
for 5 days.
The results showed an increase in cerebral perfusion as evidenced by the
client experiencing a composmentis level of consciousness, no headaches, no
anxiety, the mean arterial pressure (MAP) improved (day 1: 213 mmHg, day 2:
220 mmHg, day 3: 210 mmHg, day 4: 200 mmHg, day 5: 193 mmHg) and
intracranial pressure improved.
It can be concluded that giving a head up position of 300
is effective for
increasing cerebral perfusion in cerebrovascular accident (CVA) patients and it is
hoped that this therapy can be used in patients who are at risk of cerebral
perfusion not being as effective as nonpharmacological therapy.
