Abstract
A common nursing problem experienced by patients with anemia is ineffective peripheral perfusion, caused by low hemoglobin levels. One serious cause of anemia is upper gastrointestinal bleeding, as indicated by melena (black stools). The purpose of this case study is to provide nursing care to an anemic patient with ineffective peripheral perfusion due to melena.
The nursing care for patient Mr. T was provided, who was treated in the Mawar Ward of Dr. Harjono Ponorogo Regional Hospital for three days, from April 11-13, 2025. The research method used a case study design, with a problem-solving approach utilizing the nursing process.
The assessment results indicated that the patient experienced melena as a manifestation of gastrointestinal bleeding, with laboratory results showing a hemoglobin (Hb) level of 5.4 g/dL and an erythrocyte count of 2.42 10^/µL. Nursing care provided included circulatory maintenance and collaborative PRC (Packed Red Cell) transfusion. The evaluation revealed that the problems had not been fully resolved, such as a palpable left dorsalis pedis pulse, though still weak, and pain in the left ankle and sole of the foot.
The nursing care provided by nurses for anemic patients with ineffective peripheral perfusion should be maintained and improved to achieve more optimal and effective results. Furthermore, a more comprehensive assessment is expected to address all the patient's issues.
