Abstract
Self-care deficit is one of the common problems experienced by patients with schizophrenia, where patients face difficulties in performing daily activities such as bathing, grooming, eating, and elimination. The purpose of this scientific paper is to provide nursing care for patients with schizophrenia who experience self-care deficits, covering the stages of assessment, data analysis, nursing diagnosis, planning, implementation, and evaluation.
This research used a descriptive case study approach conducted over four days in February 2025 in Nakula Ward at Dr. Arif Zainudin Regional Mental Hospital, Surakarta. Data were collected through interviews, observation, and documentation studies. The nursing care process included assessment, nursing diagnosis, planning, implementation, and evaluation.
The assessment revealed that the patient had decreased motivation to carry out personal hygiene activities such as bathing, grooming, brushing teeth, and changing clothes. Nursing interventions included self-hygiene education, assistance with bathing and grooming, and facilitating the patient’s independence in self-care. The evaluation conducted over four days showed an improvement in the patient's ability and willingness to perform self-care independently. These results indicate that structured and consistent nursing interventions can help increase the independence of patients with schizophrenia in self-care.
The evaluation also emphasized that motivational support plays a significant role in the success of interventions. Proper nursing care can improve patient independence and have a positive impact on the quality of life of patients with schizophrenia.
