Sensory perception disorder with auditory hallucinations is a false
perception disorder that occurs due to maladaptive neurological responses. This
problem can disrupt the patient's social, emotional, and cognitive functioning and
pose a risk of dangerous behavior.
This research method uses a problem-solving approach. The nursing
process includes assessment, nursing diagnosis, intervention, implementation, and
evaluation. A case study was conducted on Mr. A from February 4 to 9, 2025.
The assessment revealed the patient talking to himself, withdrawing, and
being unable to distinguish between reality and the unreal. The diagnosis was
sensory perception disorder with auditory hallucinations. Interventions provided
included hallucination management, distraction techniques, therapeutic
communication, and collaborative antipsychotic administration The evaluation was
carried out on the fifth day when the patient was able to control hallucinations
according to the method of controlling hallucinations taught by the author.
Consistent nursing interventions demonstrated a decrease in the frequency
of hallucinations, an increase in the patient's ability to recognize hallucinatory
stimuli, and increased participation in social activities. This psychiatric nursing
care is expected to be a source of knowledge and information for healthcare
providers.
