Abstract

Coronary Heart Disease (CHD) is a leading cause of death globally and can trigger anxiety due to its life-threatening nature. Unmanaged anxiety can heighten the sympathetic response, thereby affecting the patient's physiological state. Nature-sound music therapy is one non-pharmacological intervention that can be utilized. This study aimed to implement nature-sound music therapy for a CHD patient experiencing anxiety at Dr. Harjono Regional General Hospital (RSUD) in Ponorogo.
A descriptive case study design was employed, covering the nursing process—from assessment to evaluation—for a single patient diagnosed with inferoposterior STEMI (Killip class I) and anxiety. Nursing care was administered over five days, with the therapy provided for 10–15 minutes daily in accordance with the Indonesian Nursing Intervention Standards (SIKI). Anxiety levels were evaluated using the Hamilton Anxiety Rating Scale (HARS), alongside observations of the patient's psychological and physiological responses.
The results showed that the patient initially experienced confusion and worry regarding the illness, as well as sleep difficulties; observable signs included restlessness, tension, tremors, pallor, and a trembling voice, with a HARS score of 31. Following five days of nature-sound music therapy, the HARS score dropped to 12; the patient appeared calmer and more relaxed, and vital signs stabilized.
The application of nature-sound music therapy demonstrates a reduction in anxiety levels; therefore, it can be considered a supportive non-pharmacological intervention in the nursing care of patients with coronary heart disease.
