Abstract
This final project report is titled "Continuity of Care (COC) Midwifery Care from Pregnancy through Family Planning at the Independent Midwifery Practice (TPMB) of Midwife Dina Dwi, S.Tr.Keb., Bd." Continuity of Care (COC) refers to midwifery services provided seamlessly across the continuum of pregnancy, childbirth, the postpartum period, newborn care, and family planning. Such continuous care is crucial for the early detection of complications and for improving the health status of mothers and infants, thereby contributing to efforts to reduce Maternal Mortality Rates (MMR) and Infant Mortality Rates (IMR). Midwifery care was provided to Mrs. S (31 years old, G2P10001) starting from 36 weeks of gestation through to the selection of postpartum contraception. The study employed a descriptive case study method utilizing the Varney midwifery management approach and SOAP documentation. Data collection involved interviews, observations, physical examinations, and a review of medical records. The results indicated that during the third trimester, the mother's condition remained within physiological limits, with complaints of Braxton Hicks contractions that were managed through education and monitoring. During labor, Premature Rupture of Membranes (PROM) occurred without adequate labor progression, necessitating a referral to a more fully equipped healthcare facility for further management. The postpartum period proceeded normally, with monitoring conducted over four visits. Pain at the Cesarean section incision site during the early postpartum period was managed through education on early mobilization and relaxation techniques. No complications were found in the newborn; growth and development progressed well, evidenced by weight gain that met standard benchmarks. Regarding family planning, the mother chose to use a 3-month injectable contraceptive after receiving counseling and discussing the option with her husband. Based on the care provided, it can be concluded that the implementation of comprehensive and continuous midwifery care (Continuity of Care) facilitates the monitoring of maternal and infant health, enables the early detection of complications, and supports the achievement of optimal health for both mother and baby.
