Stroke is one of the leading causes of disability in the world, causing paralysis and leading to complications. Pressure sores localized on the skin over bony prominences carry a risk of skin integrity issues. This study aims to provide nursing care for stroke patients with the nursing problem of risk for impaired skin integrity.
Nursing care for stroke patients was conducted in the Ahmad Dahlan Room at RSU Muhammadiyah Ponorogo over 5 days, from February 20-24, 2026. The method used was the nursing process approach with a back rub using virgin coconut oil.
Assessment results showed that the patient experienced physical weakness in the left limbs with muscle strength 1/1, had a compression device in place, was sweating, had decreased skin elasticity, damp skin texture, no redness, discomfort in the back, warm extremities, body temperature 36.8°C, and good tissue perfusion. The nursing problem was risk for impaired skin integrity. Evaluation results after Performed once a day for 5 days, left limb muscle strength 2/2, dressing applied, elasticity improved, skin texture improved, no redness, no back pain, extremities warm, body temperature 36.3°C, tissue perfusion good. Nursing care and back rub with virgin coconut oil once a day for stroke patients are expected to be one of the non-pharmacological therapies to prevent skin integrity problems in patients. Performed once a day for 5 days, left limb muscle strength 2/2, dressing applied, elasticity improved, skin texture improved, no redness, no back pain, extremities warm, body temperature 36.3°C, tissue perfusion good.
