Dementia or senile dementia is a syndrome of cognitive impairment that
has an impact on changes in an individual's functional, behavioral and mental
abilities when interacting in daily life. Dementia sufferers will experience social
isolation, thus affecting their quality of life. The risk of acute confusion is the risk
of
experiencing disturbances in consciousness, attention, cognition and
perception that are reversible and occur within a short period of time. The impact
of dementia on elderly people who are at risk of acute confusion results in
decreased memory, the ability to remember time, recognize people, places or
objects.
The method used is the problem solving method with a nursing process
approach including assessment, diagnosis, intervention, implementation and
evaluation. Nursing care for elderly people dementia with acute confusion risk
nursing problems for 5 days, namely 07-12 February 2024.
The results of the focus data study obtained on Mrs. M has dementia which
causes problems in the form of acute confusion because the client is over 60 years
old, causing the client to forget his age, forget time such as day, month, year and
forget people he has just met. From the results of the assessment, a nursing
diagnosis of acute confusion risk was obtained. From this analysis, intervention
was carried out, namely dementia management. After implementation, Mrs. M's
evaluation results were obtained, namely that the problem was partially resolved.
Nursing care for the elderly is expected to ensure that clients are able to
remember the day, month, year, and can memorize the names of new people and
people they already know by facilitating orientation with photo symbols. With
this, clients are able to overcome and prevent dementia and clients remember
more easily.
