THE RELATIONSHIP BETWEEN MEDICATION ADHERENCE AND HbA1c BLOOD SUGAR LEVELS IN TYPE 2 DIABETES MELLITUS PATIENTS AT PUSKESMAS SUKOSARI, KOTA MADIUN

Insulin resistance is a hallmark of type 2 diabetes mellitus (DM), which requires proper management, including regular medication use. This study aims to investigate the relationship between medication adherence and blood sugar levels (HbA1c) in type 2 diabetes patients at Puskesmas Sukosari, Kota Madiun. A correlational design with a cross-sectional approach was used, involving 45 respondents selected through a total sampling method. HbA1c levels were measured using an HbA1c Analyzer, while medication adherence was assessed using the MMAS-8 questionnaire.The results showed that the majority of respondents were female, aged 54–58 years, had a high school education, were employed, and had suffered from diabetes for 6–10 years. With a correlation value of -919** and a significance level of 0.000, bivariate analysis using Spearman’s rank test indicated a significant relationship between medication adherence and HbA1c levels. Most patients had low adherence (60,0 %) and poor HbA1c control (62.2%).Age, gender, employment status, and disease duration were key factors influencing adherence. Older patients, females, unemployed individuals, and those who had diabetes for more than ten years tended to have lower adherence levels, increasing the risk of high HbA1c levels and microvascular complications. To improve adherence and optimize glycemic control in type 2 diabetes patients, interventions such as patient education, social support, and personalized treatment programs are necessary.
