NURSING CARE FOR DIABETES MELITUS PATIENTS WITH
NURSING PROBLEMS OF SKIN INTEGRITY DISORDERS (Case Study in Mawar Room of Dr. Harjono Ponorogo Hospital)

Impaired skin integrity is a complication of Diabetes Melitus (DM), characterized by open wounds that are difficult to heal, accompanied by pain, swelling, redness, and the risk of infection. The purpose of this case study is to determine nursing care for a DM patient with impaired skin integrity, including assessment, diagnosis, intervention, implementation, and evaluation.
Nursing care was provided to Mrs. M in the Mawar Room of Dr. Harjono Ponorogo Regional Hospital for four days, April 11–14, 2025, using the nursing process. The assessment revealed a painful wound on her left toe, accompanied by pus discharge and an unpleasant odor. The wound measured 3x2 cm, appeared yellowish-red, and swollen. The nursing diagnosis was impaired skin integrity related to altered circulation.
Nursing actions included monitoring the wound for signs of infection, cleaning the wound with NaCl, removing necrotic tissue, applying a dressing appropriate to the wound type, self-education, and collaborative antibiotic administration. Evaluation showed improvement in wound condition: reduced pus and odor, healthier wound tissue, no pain, and reduced swelling and redness. Appropriate and collaborative nursing care has been proven effective in supporting the wound healing process in DM patients. Patients are advised to continue self-care and maintain blood sugar control to prevent recurrence.
