ABSTRACT
NURSING CARE FOR DIABETES MELLIRUS PATIENTS WITH NURSING PROBLEMS OF IMPAIRED SKIN INTEGRITY
(Case Study at Muhammadiyah Ponorogo General Hospital)

Impaired skin integrity is a condition where a patient is at risk of damage to the epidermis and dermis of the skin. Impaired skin integrity in patients with diabetes mellitus can have many negative impacts due to the presence of wounds such as ulcers, diabetic bullae, and gangrene, which are easily infected and cause unpleasant wounds. The purpose of this scientific paper is to provide nursing care for patients with diabetes mellitus experiencing impaired skin integrity, including assessment, diagnosis, intervention, implementation, and evaluation of nursing care.
Nursing care was performed in the Mas Mansur Room of Muhammadiyah Ponorogo Hospital on July 9-11, 2025. The method used for this nursing care is the nursing process. The assessment results showed that the patient complained of weakness and wounds that did not heal, with wounds on the legs, and the legs appeared swollen and red. Nursing actions that can be taken to address the nursing problem of impaired skin integrity are skin integrity care.
After three days of nursing interventions, the evaluation revealed that impaired skin integrity related to peripheral neuropathy was partially resolved, as evidenced by increased skin elasticity, decreased wound redness, decreased swelling, and decreased necrosis. This nursing care is expected to improve knowledge in caring for patients with impaired skin integrity.
