Abstract

Chronic Kidney Disease (CKD) is a condition in which kidney function declines
slowly and steadily, resulting in the accumulation of metabolic waste products in the body.
This decline in kidney function is characterized by a decreased glomerular filtration rate
and increased urea and BUN levels, which often impact the gastrointestinal system. As a
result, the patient's nutritional needs are disrupted, leading to nutritional problems. The
purpose of this case study is to provide comprehensive nursing care. This study aims to
understand how nursing care is provided to a patient with Chronic Kidney Disease (CKD)
with Nutritional Deficiency in the Seruni Ward of Dr. Harjono Ponorogo Regional
Hospital. Nursing care was provided for three days, from June 24 to 26, 2025, using a
nursing process approach for Mrs. J. The assessment results showed that the patient had
experienced loss of appetite for the past three days and weight loss. The patient's current
weight is 43 kg, her height is 160 cm, and her BMI is 16.8. Nursing actions provided
include: identifying the patient's readiness and ability to receive information, developing a
health education schedule based on the patient's agreement, providing opportunities for
questions, and guiding the patient in following a diet according to medical
recommendations. Based on the evaluation, it was discovered that the patient's nutritional
deficit has been partially resolved.
