NURSING CARE FOR MENTAL SCHIZOPHRENIA PATIENTS WITH NURSING PROBLEMS OF SENSORY PERCEPTION DISORDERS: VISUAL HALLUCINATIONS

Schizophrenia is a psychotic disorder characterized by the loss of the ability to distinguish between reality. One symptom is visual hallucinations, the perception of seeing things without any real stimulus. The purpose of this paper is to provide an overview of nursing care for a client with schizophrenia experiencing sensory perception disorders: visual hallucinations, through the stages of the nursing process: assessment, data analysis, diagnosis, intervention, implementation, and evaluation.
Nursing care was provided from June 3–7, 2025, in the Larasati Room of Dr. Arif Zainuddin Surakarta Mental Hospital, over four days using a nursing process approach. The assessment results indicated that the client experienced hallucinations in the form of images of a small child resembling a Dutch lady when daydreaming or alone. The client received interventions based on her needs and responses during the implementation process.
Based on the evaluation, the client showed progress in controlling her hallucinations. She was able to recognize the signs of hallucinations and respond to them by diverting attention, practicing breathing relaxation, talking with others, and taking medication regularly. These results indicate that the nursing intervention was effective and the hallucinations were under control. The active role of nurses is very important in accompanying clients through therapeutic communication and appropriate approaches.
