Application of 30-Degree Head-Up Position in Non-Hemorrhagic Stroke Patients with Nursing Problems of Decreased Intracranial Adaptive Capacity

Non-hemorrhagic stroke is one of the leading causes of disability and mortality and is often accompanied by increased intracranial pressure, which results in decreased intracranial adaptive capacity. This condition requires appropriate nursing management to prevent further complications. One non-pharmacological nursing intervention that can be applied is positioning the patient in a 30-degree head-up position. This final scientific paper aims to describe the application of a 30-degree head-up position in non-hemorrhagic stroke patients with nursing problems of decreased intracranial adaptive capacity.
The research design used was a case study with a nursing process approach, including assessment, determination of nursing diagnoses, planning, implementation of interventions, and evaluation. The subject of the study was one non-hemorrhagic stroke patient with a nursing problem of decreased intracranial adaptive capacity. The main intervention was the application of a 30-degree head-up position for 30 minutes once every 24 hours. The study was conducted in the Khodijah inpatient ward of Amal Sehat Hospital Wonogiri from November 29 to Desember 2, 2025.
The results of the intervention, which was applied consistently for five days, showed significant improvement in the patient’s condition, indicated by stabilization of blood pressure, reduction in headache complaints, increased level of consciousness, and improved neurological responses. The application of the 30-degree head-up position was proven to help improve cerebral venous return, thereby contributing to a reduction in intracranial pressure.
In conclusion, the 30-degree head-up position can be considered an effective independent nursing intervention to address decreased intracranial adaptive capacity in non-hemorrhagic stroke patients. This intervention is expected to be applied consistently in nursing practice to improve the quality of nursing care and patient clinical outcomes.
