IMPLEMENTATION OF EARLY PULMONARY REHABILITATION WITH INCENTIVE SPIROMETRY IN ADULT PATIENTS WITH COPD WITH NURSING PROBLEMS OF INEFECTIVE BREATHING PATTERNS
(Case Study in the Mina Ward, 'Aisyiyah Ponorogo Hospital)

The primary problem and most common reason COPD patients seek treatment is persistent and progressive shortness of breath. COPD patients experience inspiratory muscle weakness and/or muscle dysfunction, leading to shortness of breath. Dyspnea is a complex symptom that is the primary complaint, leading to an ineffective breathing pattern. One nursing intervention to address this problem is the implementation of early pulmonary rehabilitation with incentive spirometry. Incentive spirometry is a breathing exercise using a device to maintain maximal inspiration that provides visual feedback as the patient inhales and maintains inflation
This study used a descriptive method with a case study approach. The subject was Mr. P with COPD who presented with a nursing problem of an ineffective breathing pattern. The intervention provided was early pulmonary rehabilitation with incentive spirometry for three days, twice daily (morning and evening) for 15 minutes.
After three days of implementation, the results showed reduced shortness of breath, decreased respiratory rate, increased oxygen saturation, normal expiratory phase and no use of accessory muscles. Thus, the nursing problem of an ineffective breathing pattern was resolved.
The implementation of early pulmonary rehabilitation with incentive spirometry helps patients inhale deeply and slowly, allowing optimal alveoli expansion and increased pulmonary ventilation. With improved ventilation, the body's need for rapid respiratory compensation decreases, resulting in a decrease in the respiratory rate (RR) toward the normal range. Furthermore, increased oxygen saturation indicates improved oxygenation. Early pulmonary rehabilitation with incentive spirometry has a positive impact on the respiratory status of COPD patients.
