Abstract
Bleeding risk is a condition where a person is at risk of blood loss due to trauma, surgery, or disruption of the hemostasis process. In patients with post-operative femoral fractures, the risk of bleeding can occur due to tissue and blood vessel damage, which can lead to complications such as anemia, impaired tissue perfusion, hypovolemic shock, and delayed wound healing. The method used was a case study with a nursing process approach, including assessment, nursing diagnosis, intervention, implementation, and evaluation. Nursing care was provided at Dr. Harjono Ponorogo Regional Hospital for 3 days, from May 6, 2026, to May 8, 2026. The assessment results indicated that the client was at risk of bleeding, as evidenced by the surgical procedure, intraoperative bleeding of approximately 200 ml, bleeding in the post-operative drainage of 150 ml, pale conjunctiva, and a surgical wound on the left femur. The nursing diagnosis established was a risk of bleeding as evidenced by the surgical procedure. Interventions provided included bleeding prevention through observation, therapeutic interventions, education, and collaboration. The analysis showed that the bleeding prevention interventions provided were effective in reducing the risk of post-operative bleeding. This was evidenced by a decrease in the amount of bleeding from the drainage, from 150 ml on the first day to 10 ml on the third day. The patient's blood pressure also showed improvement, from 140/85 mmHg to 120/80 mmHg. Nurses are expected to increase their monitoring for signs and symptoms of active bleeding, such as excessive blood seepage from the surgical wound, hematoma, hematemesis, and hematuria. They are also expected to provide ongoing education to patients and their families to prevent post-operative bleeding complications and support optimal patient recovery.
