Abstract
Self-care deficit is the inability of an individual to independently perform
basic self-care activities, including bathing, grooming, dressing, eating, and
toileting. This nursing problem is commonly experienced by patients with nonhemorrhagic stroke due to neuromuscular impairment, resulting in muscle
weakness and limited mobility. If not managed appropriately, this condition may
increase the risk of infection, impaired skin integrity, reduced quality of life, and
dependence on others. This case study aimed to describe the implementation of
nursing care for a patient with non-hemorrhagic stroke experiencing a self-care
deficit at RSUD Dr. Harjono Ponorogo.
This study employed a case study design using the nursing process
approach, including assessment, nursing diagnosis, planning, implementation, and
evaluation. The subject was one patient with non-hemorrhagic stroke who
experienced a self-care deficit. Data were collected through interviews,
observation, physical examination, medical record documentation, and document
review. Nursing care was provided for three days based on the SDKI, SLKI, and
SIKI.
The assessment revealed that the patient experienced a self-care deficit due
to left-sided extremity weakness, resulting in difficulty performing personal hygiene
independently. Nursing interventions included education on the importance of
maintaining personal hygiene, assistance with personal hygiene activities, and bed
bath care. After three days of nursing care, the patient demonstrated improved
ability to perform several self-care activities, increased muscle strength, and
greater participation in meeting daily needs with minimal assistance.
The implementation of nursing care improved the patient's self-care ability.
The success of the intervention was supported by continuous exercise, patient
motivation, family support, and interprofessional collaboration throughout the
rehabilitation process.
