Abstract
Continuity of care in midwifery refers to the continuous and comprehensive provision of healthcare services, starting from pregnancy, childbirth, the postpartum period, newborn care, and continuing through to family planning programs. This care is tailored to the general health needs of women and the individual conditions of each person. Maternal and infant mortality rates are used as important indicators to assess the quality of maternal and child healthcare in a country. By providing ongoing care, it is hoped that these mortality rates can be reduced.
Midwifery care for Mrs. I, a 30-year-old woman, was carried out using the continuity of care approach, aiming to provide comprehensive care. This care utilized the Varney method and the SOAP format approach. The goal of this method is to continuously monitor the condition and development of both the mother and the fetus from pregnancy through to childbirth.
The results of the comprehensive midwifery care for Mrs. I, a 30-year-old woman, G2P1A0001, with a gestational age of 41–42 weeks, included two antenatal visits. During these visits, it was found that the fetus was in an oblique position, caused by low-lying placenta (placenta previa), and the mother experienced discomfort in the third trimester (back pain due to polyuria). Labor care began on August 28, 2024, at 10:00 PM (WIB), when the mother arrived with complaints of regular contractions accompanied by blood and mucus discharge. An internal examination revealed 2 cm cervical dilation. After 15 hours of observation, dilation had only progressed to 3 cm. On August 29, 2024, at 11:30 AM, the amniotic sac ruptured, and the fluid was mixed with blood and had a foul odor. Given this condition, a referral was made to Darmayu Hospital, Ponorogo. At 4:05 PM, the baby was delivered via cesarean section (CS), with the following measurements: female, 3,500 grams, 49 cm in length, and head circumference of 37 cm.
During the postpartum period, three visits were conducted, and all processes progressed normally. Initially, the mother chose the natural contraceptive method (LAM), and one month later decided to use an intrauterine device (IUD).
In conclusion, the results of the care show that continuity of care is effective and successful in managing pregnancy comprehensively, enabling early detection, prompt intervention, and continuous education to support the health of both mother and baby optimally.
