Abstract

Pregnancy, childbirth, postpartum, newborn care, and family planning are continuous physiological processes experienced by women. Continuity of Care midwifery services, implemented on an ongoing basis, aim to maintain quality services in accordance with midwifery care standards, thereby minimizing emergencies for both mothers and babies and helping to reduce the Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) in Indonesia. A case study approach was employed, consisting of data assessment, diagnosis formulation, planning, midwifery care implementation, evaluation, and documentation using the SOAP method. A pregnant woman diagnosed with G4P20012 was accompanied through family planning to detect potential complications early.
The midwifery care provided to Mrs. "N," 29, at 39 weeks' gestation, who attended two ANC visits for back pain and a head that had not yet entered the PAP, was successfully managed. The birth process occurred at 39 weeks gestation, on July 25, 2024, at 4:20 a.m. WIB (Western Indonesian Time). The baby was born spontaneously with the assistance of a midwife. He cried softly, had little movement, had weak muscle tone, was male, had a birth weight of 2510 grams, was 47 cm long, had a head circumference of 32 cm, and had a chest circumference of 36 cm. The third and fourth stages of labor proceeded normally, with the placenta delivered intact, and there were perineal lacerations and bleeding within normal limits. While the first and second stages were performed simultaneously, the complaints and problems that arose were well-managed. Furthermore, Mrs. N's family planning acceptor visit decided to postpone the family planning program.
Mrs. N's care was deemed successful because there were no complications or deaths for the mother or baby during pregnancy and postpartum. This success was supported by Mrs. N's cooperative attitude, which was actively involved in every stage of care, open in communication, and willing to share and express her complaints and feelings. This facilitated the researcher in providing appropriate care tailored to the mother's needs.
Continuity of Care midwifery care for Mrs. N, from the third trimester of pregnancy through delivery, postpartum, neonatal care, and family planning, has been consistently provided. Existing issues have been well addressed, resulting in the health of both mother and baby. It is hoped that Mrs. N will maintain her health by regularly attending health facilities, adopting a healthy lifestyle, and maintaining good communication with healthcare professionals. Furthermore, Mrs. N is advised to continue monitoring her baby's growth and development and participate in recommended family planning programs to maintain the ongoing health of both mother and baby.
