Abstract
The use of Unhospitalized Information System is to give informations easily, quickly and actually about Out-patient management. Therefore, it is needed to create an application Out-patient Information System to support hospital performance that manual initially turned into a computerized system to yield more detailed report. The application development was done by data collecting, system planning, system analysis, system design and database development. The application programming was developed with PHP. The database development used was MySQL and the script was developed by Macromedia Dreamweaver. It can be concluded that the design and implementation on Out-patient Information System at RSU Muhammadiyah has already been developed. Keyword: Out-patient Information System, PHP.
