APPLICATION OF PURSED LIPS BREATHING IN COPD PATIENTS
WITH INEFECTIVE BREATHING PATTERNS
(Case Study at RSU'Aisyiyah Ponorogo)

Chronic Obstructive Pulmonary Disease (COPD) is a chronic respiratory disorder characterized by airflow obstruction and often causes shortness of breath. One effective non-pharmacological intervention to reduce shortness of breath and improve ventilation is the Pursed Lips Breathing (PLB) technique. This case study aims to describe the application of the PLB technique in a 75-year-old patient, Mr. S, with a medical diagnosis of Chronic Obstructive Pulmonary Disease with acute exacerbation and a nursing problem of ineffective breathing patterns. Nursing care was provided for three days, from October 6–10, 2025, in the Muzdalifah Ward of 'Aisyiyah Ponorogo General Hospital, through a nursing process encompassing assessment, diagnosis, planning, implementation, and evaluation.
The assessment revealed shortness of breath, increased respiratory rate, use of accessory muscles, prolonged expiration, and wheezing. Nursing interventions provided included airway management, oxygen therapy, bronchodilator administration, semi-Fowler's positioning, fluid education, and the application of the PLB technique 1–3 times daily for 10–15 minutes. Evaluation after five days showed significant improvement: reduced shortness of breath, no apparent use of accessory muscles, improved respiratory rate, no prolonged expiration, wheezing, and oxygen saturation within normal limits without oxygen support.
The conclusion of this case study is that the Pursed Lips Breathing technique is effective in improving ineffective breathing patterns in COPD patients. This technique can be used as a routine nursing intervention for patients with respiratory disorders and can be taught to patients and their families for independent use at home to prevent recurrence of shortness of breath.
