Abstract

Hypertension is a condition that can cause acute pain specifically headaches
thereby compromising patient comfort and interfering with daily activities. This
study employed a case study method utilizing the nursing process comprising
assessment, diagnosis, planning, implementation, and evaluation for a single
hypertensive patient suffering from acute pain.
Nursing care was administered over an eight-day period: five days in the
Dahlia Ward at Dr. Hardjono Regional General Hospital (RSUD) in Ponorogo,
followed by three days of home visits. Assessment results revealed that the patient
was experiencing a headache radiating from the nape of the neck to the shoulders
(rated 7 on the pain scale), accompanied by facial grimacing and a blood pressure
reading of 180/80 mmHg. The established nursing diagnosis was acute pain.
Implementation was guided by the Indonesian Nursing Intervention Standards
(SIKI), specifically focusing on therapeutic actions within the Pain Management
intervention. The primary intervention involved administering Swedish Massage as
a complementary therapy four hours after the administration of ASA analgesics;
pain intensity was measured using the Numerical Rating Scale (NRS) both before
and after the intervention.
Evaluation results showed a gradual decrease in pain intensity from a scale
of 7 to 0 by the end of the intervention period. The patient appeared relaxed, showed
no signs of grimacing, was able to perform activities independently, and slept more
comfortably. Blood pressure also decreased to 132/76 mmHg. These findings
indicate that applying the nursing process with Swedish Massage as the primary
intervention can help reduce acute pain intensity and improve comfort in
hypertensive patients.
